## Forms and Requests
**Tax Form: 1099-R**
Check Your Mail: Form 1099-R is sent by Bank of America to your address of record; generally mailed by late February.
Verify Mailing Address: If you haven’t received the form by late February, contact Bank of America directly at 1-800-449-5000. Confirm your address and request a duplicate Form 1099-R.
No Electronic Access: Be aware that Form 1099-R is not available online or electronically. You’ll need to request a physical copy if you haven’t received it.
Federal Signal Pension Administration does not have access to Form 1099-R, so cannot provide copies.
